Accommodation/Tenancy Assistance

Our Team provides support to assist individuals in obtaining and maintaining appropriate housing. This may include assistance with the rental application process and fulfilling tenancy obligations as outlined in the agreement.

Assistance in Coordinating or Managing Life Stages

At Paulees Care, we take a person-centered approach and work in partnership with individuals and their families to ensure that their NDIS plans are tailored to meet their unique needs and goals. We understand the importance of providing flexible and responsive services, and our team is committed to ensuring that all individuals are treated with dignity and respect.

Our services include:

  • Assisting with the identification and engagement of appropriate service providers for direct support
  • Coordinating assessments and training with relevant professionals to support the individual’s needs and goals.
  • Facilitating the development of service agreements between the individual and chosen service providers
  • Providing guidance on the NDIS pricing guide and assisting with financial management
  • Offering support for problem-solving and decision making
  • Educating individuals and families on their rights and responsibilities under the service agreement

Facilitating any necessary changes or amendments to the service agreement in line with the individual’s needs and goals.

Assistance with Daily Personal Activities

We work alongside you to develop skills that will enable you to maintain your independence and support you while you navigate your choices. Our team will work with our participants, their families, and carers to identify strengths and goals.

This includes:

  • Assistance with shopping
  • Accessing Centrelink
  • Assistance with cooking and meal preparation
  • Support with cleaning
  • PEG Feeding
  • Continence assessment and care
  • Sleep-over supports.
  • Accessing Public Trust and other services
  • Paying bills, setting routines, and meeting other responsibilities.
  • High-intensity self-care assistance (dressing, showering, toileting, continence support, grooming, administering medication, feeding support)

Assistance with Travel/Transport Arrangements

We can help you to travel independently on public and private transport to stay connected with family, friends, and the community. Having the independence to get out and about in your community is important to have the freedom to do the things you enjoy. We can support you to travel on the bus, train, ferry, or in your car is a great way to visit friends and family, travel to your job, do some shopping, and access health and education services.

  • Access to local community groups of interest.
  • Assistance in utilizing transport services.
  • Provide suggestions to manage personal barriers.
  • Supporting you with getting a GO Card and topping up credit
  • Using Maps, Apps, and Timetables to help you get where you need to go.
  • Work with you to identify travel and transport goals.

Innovative Community Participation

Socialization and being connected to a community can contribute to the feeling of empowerment of people with disabilities. It is good for the general mental health and outlook towards life.

Paulees Care provides access to mainstream services that promote the integration of individuals with disabilities into their communities. Our goal is to expand opportunities for community engagement and employment for these individuals, thus improving their overall quality of life.

Paulees Care offers personalized training and development programs that focus on building life skills. These programs include training and support for using public transportation and participating in community, social, and recreational activities. Our goal is to empower individuals to gain independence and improve their quality of life.

Participation in Community, Social and Civic Activities

Our community participation programs are designed to help participants to socialize, make connections, and build a network of friends and/or groups.

Our team can arrange transport, accompany you and provide any personal support you require while you are out.

We can support you to:

  • Find opportunities to volunteer.
  • Go shopping.
  • Go to the local club.
  • Join a sports club.
  • See a movie or play.
  • Try a local hobby or craft group.
  • Try a new cafe.
  • Visit the library.
